What to include
An affidavit must contain: the deponent's full name, address, and occupation; the court and proceeding details; numbered paragraphs of facts within the deponent's personal knowledge; identification of documents (annexures or exhibits); and the jurat (the witnessing clause). Hearsay evidence must be identified as such (e.g., "I was told by [person] that...").
